High School Sleepover at United Methodist Church (UMC) and Grace Lutheran Church (at the Grace Center)

June 5 at 7:30pm at the UMC –end on June 6 at 8:30am at the Grace Center

*this included the students who finished 8^th grade and are enrolled for high school next year.

Please note: The student must remain for the entire event, unless a leader is notified that a parent is picking them up early.

                By signing this form the parent, guardian or individual agrees to assume and accept al risks and hazards inherent in participating in the High School Sleepover Event. They also agree to not hold Grace Lutheran Church and /or its owners, agents, or employees liable for damages, losses, or injuries to the person(s) or property undersigned.
